Part 1: From Care Delivery to Measurable Value: Aligning SNF Teams Around the Metrics That Now Define Success
Kim Saylor, Chief Growth Officer, shares how Skilled Nursing Facilities are now judged by measurable outcomes, not just care delivered, and why aligning teams around key performance metrics is critical. Learn how making these metrics part of a weekly operating rhythm drives consistency and results in value-based care.
Part 2: From Insight to Execution: An Operating Model for Driving Measurable SNF Performance
In part 2, Kim shares how SNFs can turn aligned performance metrics into operational improvement using a 30/60/90-day execution model. She outlines how focusing on one performance gap, building structured weekly workflows, and standardizing change over time drives measurable outcomes and strengthens referral and payer relatinships.
Question: When your SNF QRP Provider threshold report % is getting close to 90% what report do you run to see what is triggering them?
Answer: There are 2 ways - you can review validation reports that have been submitted or you can run MDS Error report to see what reports have issues. To run an error report, you have to accurately identify the payment reduction error codes per CMS. The error codes can be located in the Minimum Data Set (MDS) Error Message Reference Guide. Current payment reduction error codes are -3897, -3808 and -3908 but it is advisable to check the Error Message Reference Guide for updated information.
Responses reflect general regulatory guidance and may vary by state, payor, or circumstance.
